Job summary

Care at Home in Massachusetts is seeking a Home Health Registered Nurse to manage and deliver comprehensive home health services to patients in Winchester and Medford. The role emphasizes assessments, interventions, and coordination with physicians to avoid unnecessary hospitalizations.

The ideal candidate holds an active RN license, a valid driver’s license, and at least 1 year of acute care nursing experience, with BLS. Availability for evenings, weekends, and holidays is required.

Qualifications

• Active RN license in the state of service; licensure must be current.
• Valid state-issued Driver’s License.
• One year of acute medical/surgical nursing experience.
• Basic Life Support Certification.
• CPR certification required if hired before 9/28/2026; BLS within 30 days if after hire.

Responsibilities

• Assesses patients' physical and psychosocial needs following established Nursing Practice standards.
• Develops plan of care with patient, family, and health care team.
• Documents patient data and communications in the patient record.
• Completes physicians' orders and OASIS on all patients.
• Coordinates care and discharge planning with the team.
