Job summary

Tufts Medicine in Lowell, MA is seeking a Home Care Nurse to provide professional and nonprofessional nursing care in patients' homes per physician orders. You will triage calls, coordinate care with Clinical Managers, and ensure timely delivery of services to patients and families while adhering to regulatory standards.

You must hold an RN license and have strong community health experience; prior telephone triage is preferred. This is a full-time role offering 40 hours per week.

Qualifications

• Registered Nurse (RN) required.
• Three (3) years of community health experience or 1 year acute med/surg plus 2 years home health.
• Telephone triage experience preferred.

Responsibilities

• Coordinate clinical services to meet agency objectives.
• Provide direct patient care and timely services for patients and families at home.
• Communicate with Clinical Managers and Clinical Director regarding practice and performance.
• Demonstrate clinical expertise across care methods, infection control, and regulations.
• Assist with orientation and preceptor duties for clinical staff.
• Guide staff on visit frequency and homebound status.
• Evaluate staff performance with the clinical manager.
• Ensure coverage based on census, skill mix, and patient needs.
• Triages physician calls and follows up with field staff.
