Job summary

WelbeHealth is seeking a Home Health Manager to lead a skilled home care team, oversee nursing in participants’ homes and in nursing/board & care facilities, and coordinate therapy services. The role focuses on delivering high-quality care within a value-based model and ensuring adherence to care plans.

The ideal candidate has a California RN license and at least 2 years of home health experience, with a bachelor’s degree preferred.

Qualifications

• Bachelor’s Degree preferred; substitution allowed for relevant experience
• Minimum of 2 years of nursing experience in a home health setting with frail/elderly population preferred
• California Registered Nurse (RN) License required
• Knowledge of nursing to treat frail, elderly participants and handle complex clinical scenarios

Responsibilities

• Direct a high-performing home care department ensuring workflows and policies are followed at all times
• Guide and maintain home care staff, including hiring, training, and coaching all team members
• Assess the effectiveness of home health nursing assessments and IDT participation
• Ensure home care nurses deliver and document nursing interventions as agreed in care plans
• Oversee the quality of care provided to participants in nursing homes and board and care facilities
