Job summary

WelbeHealth is seeking a Home Care Coordinator to coordinate in-home care assessments, schedule and oversee personal care for participants in Stockton, CA. This role supports the Home Care Manager and collaborates with nursing, therapy, aides, social services, and physicians.

The position offers hybrid work, on-site IDT meetings and home visits, a comprehensive benefits package, base pay plus bonus, 401(k) match, and PTO.

Qualifications

• Healthcare licensure or equivalent required; minimum 3 years professional experience
• Minimum 3 years of case management or nursing experience in clinical/home setting with frail/elderly population
• Nursing knowledge/training to treat frail, elderly participants preferred

Responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders, participant questions, and referrals
• Communicate with participants by phone and coordinate with nursing, therapy, aides, social services, and physicians regarding schedule changes, test results, and other related updates
• Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling hours
• Partner with Marketing to support enrollment of prospective participants into the program
• Assist with staffing and scheduling activities, soliciting, and manager input
• Participate in end-of-life care coordination and support
