Ennoble Care is seeking a full-time Care Coordinator, Embedded to operate from a senior living facility in Westwood, NJ. You will ensure high-quality care by coordinating with clinicians, staff and families, and managing caregiver schedules in Ennoble Care’s EMR system.

You should be compassionate, well-organized, and comfortable on the phone, with strong notes and documentation skills. This in-office role emphasizes teamwork and excellent customer service.

Qualifications

• Must be comfortable with speaking on the phone for long periods.
• Be compassionate and empathetic toward patients and families.
• Capable of accurate note-taking and timely documentation.
• Ability to multitask and stay organized in a fast-paced setting.
• Full-time, in-office role in Westwood, NJ; proficiency with Outlook and MS Office.
• Knowledge of basic healthcare terms and care principles.

Responsibilities

• Complete individualized patient care plans and perform care coordination.
• Maintain frequent contact with patients to support programs and outcomes.
• Document all client communications accurately.
• Coordinate with Primary Care Clinician and care team on needs and plans.
• Inform Team Supervisor of issues related to the care plan and execution.
• Collaborate across departments to meet patient needs and family involvement.
• Utilize technology including EMR, secure email, and telephone systems.
• Adhere to documentation protocols and daily work logs.
