Job summary

Home Instead is seeking a compassionate Client Care Advisor to help older adults live safely and comfortably at home. You will serve as the client’s advocate, build strong relationships with families, and coordinate all aspects of client care, from initial consultations to ongoing service plan adjustments.

You will work with Care Pros and the care team to deliver personalized plans, conduct quality assurance visits, and ensure exceptional standards of care.

Qualifications

• Prior experience in client services, healthcare, senior care, social work, or care coordination is strongly preferred.
• Experience with DHHS Home care services and waivers programs is a plus.
• Availability to work evenings or weekends as needed.
• Valid driver’s license and reliable transportation (if client visits are required).
• Proficiency in Microsoft Office Suite and client management software is preferred.

Responsibilities

• Serve as trusted advocate for clients and families, ensuring high-quality care.
• Be primary contact for clients and families, building lasting relationships.
• Conduct care consultations, assess needs, and develop tailored care plans.
• Coordinate care with Care Pros, schedulers, and the team to ensure seamless service.
• Perform quality assurance visits and update client records.
• Lead introductions between clients and Care Pros and manage documentation.
