Job summary

Cornerstones of Care is seeking a Lead High Acuity Case Management Specialist to join our team. This role focuses on mentoring and developing high acuity case managers and behavioral interventionists while elevating service delivery for youth and families.

The position involves coaching, quality assurance through case reviews and AVATAR documentation, and working with the Manager of High Acuity Permanency Services to improve outcomes. Expect a collaborative, mission-driven environment.

Qualifications

• Bachelor's degree in social work, criminal justice or other human services related field.
• Master's degree in social work and licensed by the Behavioral Sciences Regulatory Board to practice in Kansas is preferred.
• At least 21 years of age and pass background check, physical, and drug screening.
• A valid driver's license, proof of current vehicle insurance, and reliable transportation.

Responsibilities

• Mentor up to 2 high acuity foster care case management specialists and 2 behavioral interventionists with on-the-job coaching.
• Assist with peer record reviews, complete documentation audits, and evaluate reports to meet contractual requirements.
• Serve on program and system improvement committees and participate in strategic planning and organizational development.
• Analyze and assess each family and child situation to recommend supportive services and resources (education, medical, therapy, independent living).
• Cover for managers in their absence as needed and manage a caseload of 10–12 youth in the High Acuity program.
