Qualifications

• Bachelor’s degree in social work or other related field is required.
• At least 21 years of age and pass background check, physical and drug screening
• A valid driver’s license in the state you reside in, proof of current vehicle insurance and reliable transportation

Responsibilities

• Engage in family finding to identify and take advantage of relative/kinship placement options for children in care
• Continually analyzes and assesses each family and child situation on an individual basis, using advanced knowledge in child welfare to develop recommendations regarding supportive services and resources each child and family needs
• Facilitate service planning with the child, family, and other treatment team members in accordance with Cornerstones of Care and DCF policy
• Coordinate any supportive services and resources children under case management services may need to ensure timely and efficient service delivery
