Job summary

Cornerstone Montgomery is seeking a Case Manager to develop individualized rehabilitation plans for clients with mental health and co-occurring disorders. The role includes documenting progress, ensuring compliance with standards, and coordinating home visits at least twice a month to assess client needs.

Qualifications

• Bachelor's degree required in Social Work, Psychology, Rehabilitation Counseling, or related field.
• At least 1 year of experience in human services preferred.
• Clean driving record with no more than 2 points.
• Ability to pass a criminal background check.
• Must work a minimum of 20 hours per week to be eligible for benefits.

Responsibilities

• Develops an Individual Rehabilitation Plan (Treatment Plan) with the person served to identify case management needs in advocacy, coordination of services and resource procurement.
• Documents communication and interactions with the person served in progress notes in a timely manner.
• Ensures assigned cases and all documentation are compliant and accurate according to agency, COMAR, and CARF standards.
• Schedules and conducts home visits with persons served to monitor their condition and assess needs at least twice per month.
• Provides linkages and access to resources, entitlements and supports to ensure successful community integration.

Education

Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or related discipline
