Job summary

Acadia Healthcare is seeking a dedicated Case Manager to assess, plan, implement, coordinate, monitor, and evaluate treatment services for residents. You will develop care plans, collect clinical data, and coordinate across inpatient and outpatient settings to ensure continuity of care.

Applicants should have a Bachelor’s degree in Health Admin, Social Science or related field, with strong interagency liaison and knowledge of mental health/substance use fields preferred.

Qualifications

• Bachelor’s degree in Health Administration, Social Science or related field.
• Ability to liaise with community agencies to ensure continuity of care.
• Knowledge of mental health and substance use fields preferred.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate services during treatment.
• Develop and monitor individual care plans; collect clinical data.
• Coordinate care by tracking inpatient or outpatient services.
• Convey medical criteria between client/patient and insurance provider and treatment team as needed.
• Conduct precertification with third party payers and negotiate approved days for admission.
• Assigned tasks for admission review, concurrent review and discharge planning.
• Perform other functions and tasks as assigned.
