Job summary

Urban Alchemy in Portland, OR is seeking a Manager, High Acuity Care Coordination to mentor the Care Coordination team on the front lines. You will support guests facing complex behavioral health challenges while advancing pathways to stability and housing.

The role requires trauma-informed practice, collaboration with medical and social service partners, and strong organizational skills to ensure high-quality care and grant compliance. A Bachelor’s in a related field is preferred.

Qualifications

• 3+ years in social services, street outreach, or intensive case management, especially with high-acuity populations.
• Deep knowledge of local service providers, CoC, and referral pathways.
• Proficiency in trauma-informed care, harm reduction, and de-escalation strategies.

Responsibilities

• Provide on-site mentorship to Care Coordinators, modeling de-escalation and trauma-informed communication.
• Lead intensive case navigation for complex high-acuity guests; develop stabilization plans.
• Facilitate structured case conferences with internal and external partners to align goals and care plans.
• Liaise between field teams and external providers to advocate for equitable guest access.
• Oversee documentation quality and ensure compliance with program and grant requirements.
• Coach the team in client-centered care, crisis intervention, and mental health awareness.
