Job summary

Cornerstones of Care in Kansas City, KS is hiring a Lead High Acuity Case Management Specialist. You will mentor and coach a small team, perform documentation audits, and participate in program improvement to enhance foster care services.

You will use Signs of Safety to guide service planning for 10–12 youths and report to the Manager of High Acuity Permanency Services. Minimum qualifications include a Bachelor’s in a human services field, a Master’s in social work with licensure preferred, and

Qualifications

• Bachelor’s degree in social work, criminal justice or related field.
• Master’s degree in social work and licensure preferred in Kansas.
• 21+ years of age with background check, physical, and drug screening.
• Valid driver’s license, proof of insurance, reliable transportation.

Responsibilities

• Provides mentoring, coaching, and training to up to 2 high acuity foster care case management specialists and 2 behavioral interventionists.
• Performs peer record reviews, documentation audits, and evaluates reports to meet contractual requirements.
• Serves on program and system improvement committees and participates in strategic planning.
• Analyze and assess each family/child situation to develop recommendations for services (education, medical, therapy, independent living).
• Assumes manager responsibilities in their absence as needed.
• Cares a caseload of 10–12 youth under the Foster Care Case Management Contract using SOS, including discharge cases.
