The position supports a geriatric outpatient clinic with opportunities for memory evaluations, diabetes education, and chronic disease management within a system-wide Epic EMR environment.

Qualifications

• Board-certified NP or PA with geriatric focus.
• Oregon NP license (or ability to obtain during credentialing).
• BLS/ACLS/PALS certification within 30 days of hire.

Responsibilities

• Provide comprehensive primary and consultative care for adults 65+.
• Collaborate with a multidisciplinary team to improve quality of life for seniors.
• Serve as primary care provider and consultant in a geriatrics-focused SHWC.
