Responsibilities

• Provide in-clinic visits and off-site community visits to patient residences (including assisted living facilities, SNFs, and homes).
• See 4–6 patient encounters per shift in an outpatient setting.
• Work within an 11-person interdisciplinary care team and collaborate with nurses on off-site visits.
