Responsibilities include clinic visits, follow-ups, and care coordination, typically handling about 16 patients per day. Schedule is Mon–Fri with a flexible workday, potentially remote work. Benefits include full insurance and retirement options.

Qualifications

• Graduate of accredited Physician Assistant/Nurse Practitioner Program.
• Master degree required and NCCPA certification.
• Active PA license and DEA registration.
• Active certification as Adult NP with licensure.
• Experience as PA/NP in post-acute/geriatric care.

Responsibilities

• Work with a Skilled Nursing Facility.
• Clinic visits, follow ups, established visits.
• Develop treatment plans and coordinate care.
• Manage an average of 16 patients per day.
