Job summary

Pacific Medical Specialty Group is seeking a compassionate Nurse Practitioner or Physician Assistant to provide care for residents in skilled nursing and long-term care facilities in Eugene, Oregon. The role offers a flexible daytime schedule, clinical autonomy, and collaborative physician support to manage complex geriatric cases.

The position focuses on comprehensive patient care, ongoing assessments, and coordination with facility teams to improve quality of life and reduce hospital visits.

Qualifications

• Graduation from an accredited Nurse Practitioner or Physician Assistant program.
• Active, unrestricted Oregon license, or ability to obtain one.
• National board certification or board eligibility by start date.
• DEA registration and prescriptive authority, or ability to obtain them.
• National Provider Identifier (NPI).
• Eligibility to participate in Medicare and Medicaid.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Manage ongoing medical needs of residents in Eugene-area skilled nursing and long-term care facilities.
• Collaborate with physicians and interdisciplinary teams to deliver individualized care.
• Develop treatment plans and prescribe medications within scope of practice.
• Order and review labs and imaging; address results promptly.
• Provide post-acute and transitional care after hospital visits.
• Educate residents, families, and caregivers about diagnoses and care plans.
• Coordinate care with collaborating physicians and other specialists.
• Support efforts to reduce avoidable ED visits and hospital readmissions.
