Job summary

Mayo Clinic Senior Services seeks a Registered Nurse to deliver care coordination and triage for geriatric patients by phone and in person, primarily in home or assisted living settings within 20 miles of Rochester.

You will participate in nursing planning, education, and transitions of care, using Mayo Clinic Nursing Professional Practice Model, and may mentor others while rotating through days, evenings, and weekends with potential on-call shifts.

Qualifications

• RN with baccalaureate degree or equivalent; licensure required.
• One year of RN experience in applicable care setting.
• Excellent communication and teamwork skills.
• Computer skills; EMR experience preferred.
• Ability to work flexible hours including nights and weekends.

Responsibilities

• Coordinate nursing care, direct patient care, and education.
• Deliver care coordination and triage for geriatric patients by phone and in person.
