Job summary

Mayo Clinic is seeking a Registered Nurse for Senior Services in Rochester, MN. The RN delivers care coordination and triage, conducts assessments and education in patients' homes or nearby facilities, and supports transitions of care across the care continuum.

The role requires a BSN (or higher) and an active RN license; one year of RN experience; and the ability to work flexible hours, including weekends. Driving is required with mileage reimbursement; travel up to 20 miles is expected.

Qualifications

• RN with BSN or higher and applicable state licensure
• 1 year RN experience in appropriate care setting or 1 year LPN experience at Mayo Clinic
• Excellent communication skills and teamwork
• Proficiency with EMR systems and ability to adapt to flexible hours

Responsibilities

• Coordinate nursing care and patient education across care settings
• Provide assessments and education in patients' homes or assisted living facilities within 20 miles of Rochester
