Job summary

Mayo Clinic in Rochester, MN seeks a Registered Nurse in Senior Services to deliver coordinated nursing care, education, and triage for geriatric patients. Responsibilities include home-based assessments and education within 20 miles of Rochester, with mileage reimbursement for using a personal vehicle.

The RN will lead across the nursing practice model, support transitions of care, and demonstrate leadership, communication, and critical thinking in a dynamic team environment.

Qualifications

• Graduate of a baccalaureate nursing program recognized by ACEN, CCNE, or NLN CNEA.
• At least one year of RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic.
• Excellent communication skills (verbal and written) and ability to work in a team.
• Computer skills with electronic medical record systems preferred.
• Flexibility to work days, evenings, nights, holidays, weekends, and on-call.

Responsibilities

• Coordinate nursing care, provide direct patient care and education.
• Perform care coordination and triage via phone and in person for geriatric patients.
• Educate patients and families and facilitate transitions of care.
• Lead and mentor within Mayo Clinic Nursing Practice Model; delegate tasks per patient needs.
• Possess strong communication and critical thinking; may participate in committees or projects.
