Qualifications

Responsibilities

• Perform assessments and patient education in homes or assisted living facilities within 20 miles of Rochester.
• Coordinate nursing care and transitions, lead in quality improvement, and mentor team members.
• May drive personal vehicle with mileage reimbursed; driving may be part of duties.
