Job summary

Mayo Clinic is seeking a Registered Nurse in Senior Services to coordinate nursing care for geriatric patients, including assessments, education, and transitions of care. Qualifications

• Graduate of a baccalaureate nursing program and eligible for RN licensure.
• One year of RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic is required.
• Excellent communication and teamwork skills; experience with EMR systems preferred.

Responsibilities

• Coordinate nursing care, including direct patient care, education, and transitions of care.
• Deliver assessments and education to geriatric patients in home or assisted living facilities within 20 miles of Rochester, MN.
• Drive own vehicle as needed with mileage reimbursement; valid driver’s license required.
