Job summary

Mayo Clinic is seeking a Registered Nurse for Senior Services to coordinate nursing care, provide patient education, and support transitions of care for geriatric patients.

Work involves assessments in patients’ homes or assisted living facilities within 20 miles of Rochester. A BSN, RN license, and flexible hours are required; driving own vehicle with mileage reimbursement is provided where applicable.

Qualifications

• Graduate of a baccalaureate nursing program with ACEN/CCNE/NLN CNEA accreditation or equivalent.
• One year RN experience in an applicable care setting.
• Current RN license; Basic Life Support (BLS).
• Excellent communication skills, verbal and written.
• Experience working in a team environment; mentoring possible.
• Ability to work flexible hours including days, evenings, nights, weekends and holidays.

Responsibilities

• Provide nursing care coordination and triage for geriatric patients via phone and in person.
• Deliver assessments and education to patients in their homes or nearby facilities within 20 miles of Rochester.
• Mentor and lead within Mayo Clinic Nursing Professional Practice Model; participate in committees or projects.
