Qualifications

  • Master’s degree preferred; Bachelor’s degree required in social work, early childhood, marriage and family therapy, or another related field, or an equivalent combination of education and experience.
  • Minimum 3 years of experience providing case management, family support, community engagement, or service coordination.

Responsibilities

  • Provides advanced case management and coordination to support families in housing, education, employment, economic stability, and well-being.
  • Delivers ʻohana support through assessments, service plans, coaching, and information sharing to help families achieve goals.
  • Facilitates activities, groups, and structured learning experiences.
  • Designs and facilitates curriculum integrating LT values, cultural grounding, and child development frameworks.
  • Collaborates with LT teammates and community partners to meet family needs and reflect community context.
  • Participates in program planning, budget development, and evaluation.
  • Maintains timely documentation and electronic records in accordance with LT policies.
