SUMMARY OF DUTIES

Provides case management and coordination services to youth who are referred and their families. Orients youth and their families to the program. Assists by providing feedback to youth and family driven service plans. Provides warm handoffs during referrals and coordination.

  • Provides family life education and advocacy as needed and make every reasonable effort to make certain that the services are flexible to the families.
  • Appropriately and selectively utilizes a broad variety of skills and techniques to engage youth and families in services.
  • Performs required administrative tasks related to caseload, such as correspondence, reports, and maintaining of case records.
  • Works with youth, designated schools), and other community members.
  • Provide transportation for families on occasion using personal or agency vehicles as needed

** This position may have a set or flexible shift schedule as part of a 24/7 on-call rotation.

EDUCATION AND TRAINING REQUIREMENTS

  • Four-Year College Degree from a school accredited by a recognized accrediting agency - general major. Requires basic knowledge or training in a specialized or technical field or trade, such as social work, professional accounting, finance, business administration, and marketing.
  • Other (Specify): Master’s Degree preferred. Knowledge of a specialized field or may lead to licensing (human services), or formalized apprenticeship program preferred

EXPERIENCE

  • Over two years, up to and including four years
  • Other: Two years' experience working with at-risk youth, preferably youth who have been involved with the juvenile justice system.

SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

  • Participates in the implementation of a trauma-informed approach to education and behavior management, that promotes relationship building, pro-social behaviors, cooperative learning, self-help skills, academic achievement, and strengthening daily living skills within an academic environment.
  • Excellent written and communication skills

  • This job requires a valid Hawaii driver's license and a clear driver's abstract and continued employment may be contingent on maintaining a clear driver’s abstract.

  • Requires contacts both inside and outside at all organizational levels. Requires considerable tact, discretion and persuasion to obtain results. May present new methods, programs, and controversial issues. Improper handling may affect operating results and the delivery of participant services.
