Job summary

Parents and Children Together in Honolulu seeks a Case Manager to support families through voluntary, strengths-based services focused on child safety. You will work in home or natural environments, coordinating services and developing safety plans with families identified as moderate to high risk.

The role requires a Bachelor’s degree in social work or related field, a valid driver’s license, and ability to obtain background checks.

Qualifications

• Requires a Bachelors’ degree in social work, psychology or related human services field.
• Must possess and maintain a valid driver’s license and daily access to an automobile.
• Able to obtain and maintain required background checks (criminal, CWS, National Sex Offender Registry).

Responsibilities

• Ensures service delivery meets funding, state, and organizational standards.
• Provides assessment and lead case management to connect families with resources.
• Develops goals with the Family Partnership Plan and tracks progress.
• Provides parenting and family strengthening services to reduce risk factors.
• Links families to community resources and coordinates services for safety.
• Meets monthly with supervisor to discuss progress and crises as needed.
• Completes timely, accurate documentation and reporting for supervision.

Education

Bachelor's degree in Social Work, Psychology, or related human services field
