Job summary

NorthWest Senior and Disability Services seeks a skilled Case Manager to support older adults and individuals with disabilities. The role involves assessing needs, developing care plans, and coordinating services to promote health, safety, independence, and quality of life.

You will work directly with consumers, collaborating with community partners to connect people with resources. Strong written and verbal communication, organization, and confidentiality are essential in this role.

Qualifications

• Bachelor’s degree in social sciences or related field.
• 4+ years of education and/or experience combining knowledge and practice in social services.
• Experience working with consumers in social service settings or similar.
• Successful completion of a language proficiency test.

Responsibilities

• Meet with consumers to assess service needs and daily living activities.
• Develop individualized care plans in accordance with program requirements.
• Coordinate services such as in-home care, transportation, medical supplies, and community resources.
• Monitor consumer needs and update care plans as circumstances change.
• Complete required assessments, documentation, and case records in Agency and State systems.
• Work with Agency staff, providers, healthcare partners, and community organizations to support consumers.
• Maintain confidentiality and comply with policies and reporting requirements.
• Provide respectful and responsive customer service to consumers, providers, and partners.
