Job summary

NorthWest Senior and Disability Services (NWSDS) is seeking dedicated Case Managers to assess service needs, develop care plans, and coordinate in-home and community resources for older adults and individuals with disabilities in Salem, OR. You will work closely with the Program Manager and collaborate with partners to ensure health, safety, independence, and quality of life.

Qualifications

• Minimum four (4) years of education and/or experience demonstrating relevant knowledge, skills and abilities.
• Bachelor's degree in social sciences or related field.
• Experience or volunteer work with consumers in social service settings; education can be substituted.
• Successful completion of a language proficiency test.

Responsibilities

• Meet with consumers to assess service needs and daily living activities.
• Develop individualized care plans per program requirements and consumer needs.
• Coordinate services such as in-home care, residential services, transportation, medical supplies, and community resources.
• Monitor consumer needs and update care plans as circumstances change.
• Complete assessments, documentation, and case records in required systems.
• Work with staff, providers, healthcare partners, and community organizations to support consumers.
• Maintain confidentiality and comply with policies and reporting requirements.
• Translate and explain program information for non-English speakers; translate materials as needed.
