Job summary

NorthWest Senior and Disability Services (NWSDS) is hiring a Case Manager to assess service needs, develop care plans, and coordinate a range of supports for older adults and people with disabilities in Oregon. The role involves working with consumers and partners to connect them with resources that promote health, safety, independence, and quality of life.

The ideal candidate will meet minimum qualifications, communicate effectively, maintain confidentiality, and adapt plans as needs evolve.

Qualifications

• Bachelor’s degree in social sciences or related field.
• Four years of experience in social services with consumers.
• Successful completion of a language proficiency test.
• Experience with seniors, adults with disabilities, or other vulnerable populations is a plus.

Responsibilities

• Meet with consumers to assess service needs and daily living activities.
• Develop individualized care plans per program requirements and consumer needs.
• Coordinate services such as in-home care, residential services, transportation, and community resources.
• Monitor and update care plans as circumstances change.
• Complete assessments, documentation, and case records in Agency and State systems.
• Collaborate with staff, providers, healthcare partners, and community organizations.
