Responsibilities

• Meet with consumers to assess needs and identify support options.
• Develop individualized care plans per program requirements.
• Coordinate services such as in-home care, transportation, medical supplies.
• Monitor and update care plans as circumstances change.
• Maintain required assessments, documentation, and case records inAgency and State systems.
• Collaborate with staff, providers, healthcare partners, and community organizations.
