Job summary

NorthWest Senior and Disability Services (NWSDS) is seeking a full-time Case Manager in McMinnville, Oregon to assess service needs, develop care plans, and coordinate in-home and community support for older adults and people with disabilities.

The role works with partners, maintains confidential records, ensures compliance, and provides respectful customer service. Starting salary is $4,879 per month, with strong benefits including health, retirement, PTO.

Qualifications

• Bachelor’s degree in social sciences or related field.
• Experience with social services environments is preferred.
• Four years of combined education/experience or equivalent.
• Completion of a language proficiency test is required.

Responsibilities

• Meet with consumers to assess service needs and daily living activities.
• Develop individualized care plans per program requirements.
• Coordinate services including in-home care, transportation, and community resources.
• Monitor and update care plans as needed.
• Document assessments and case records in agency/state systems.
• Collaborate with staff, providers, and community partners to support consumers.
• Communicate in the identified language pair and translate materials as needed.
