Job summary

Tri Valley, Inc. seeks a Case Manager for Senior Care Options to work with an interdisciplinary team to coordinate medical, behavioral, and long-term services for SCO enrollees based on individual needs.

Responsibilities include entering referrals into SAMS, conducting home visits, developing care plans, coordinating community LTSS, and communicating with the care team to ensure timely updates and disenrollment when appropriate.

Qualifications

• Bachelor's degree in Social Work or Human Services is required.
• Valid Driver's license and ability to maintain insurance and a driving record check.
• Strong computer skills and proficient in documentation and record-keeping.
• Experience working with people with disabilities and/or elders in need of LTSS is preferred.

Responsibilities

• Enter new referrals into SAMS from One Care programs and SCO rosters.
• Complete home visits and assess long-term services needs for enrollees.
• Send initial assessment to One Care staff and update data in One Care databases.
• Develop care plans for community LTSS to improve or maintain health and function.
• Arrange and coordinate community LTSS after care plan approval.
• Monitor provision and effectiveness of services per care plan.
• Document communications with the care team and update cases.
• Close cases in SAMS according to disenrollment dates.
