Job summary

Capital Health Care Network is seeking a proactive Home Care Coordinator to manage patient transitions from facilities to our home health services. You will work with the marketing team to drive census growth through relationships and community events, hosting facility activities and guiding families through discharge.

The role emphasizes non-clinical coordination, strong communication, and travel between facilities to ensure smooth transitions to home health care.

Qualifications

• Experience in health care settings, especially senior living or home health care.
• Strong ability to build relationships with facilities, case managers, and families.
• Excellent communication, planning and organizational skills.

Responsibilities

• Facility Relations & Outreach: drive referral growth and hit agency census targets.
• Patient Intake & Family Support: meet patients/families and present home health options.
• Care Transition Coordination: coordinate transfers and ensure smooth discharge to home.
