Job summary

Tanner Health System is seeking an experienced Lead Care Coordinator to coordinate patient transitions and optimize care pathways. The role requires strong nursing expertise, care management knowledge, and the ability to lead a multidisciplinary team in a dynamic hospital environment.

The Lead Care Coordinator will train staff, ensure adherence to policies, and collaborate with physicians, families, and community resources to deliver safe, efficient discharge planning and quality outcomes.

Qualifications

• Two years of related experience.
• Excellent verbal and written communication skills.
• Demonstrated professionalism and integrity.
• Ability to organize, prioritize, and make decisions with minimal supervision.
• Knowledge of care coordination principles and regulatory standards.

Responsibilities

• Coordinate care transitions for patient populations with the care team.
• Mentor and train new staff and support ongoing education.
• Serve as a liaison with physicians, families, and external agencies.
• Assist Care Coordination Manager with orientation for new hires.
