Job summary

Bay Area Community Health (BACH) seeks a Community Health Worker III to support patients across Alameda and Santa Clara Counties. The role involves outreach, care coordination, and linking patients to community resources, housing, food, and transportation assistance.

The CHW III will liaise with primary care and specialty teams, work at multiple sites, and may require evenings or weekends. Strong HIPAA understanding and program compliance are essential.

Qualifications

• High school diploma or equivalent.
• 2-4 years of experience in a community-based health care service capacity preferred.
• Bilingual abilities preferred; Spanish skills helpful.

Responsibilities

• Coordinate and deliver training for CHW I through CHW II.
• Assess patient’s home living situation.
• Conduct housing and crisis assessments.
• Conduct SDOH assessments and provide resources as appropriate.
• Ensure program compliance with applicable rules and procedures.
