Job summary

Bay Area Community Health seeks a Community Health Worker III (CHW III) to serve and assist patients across Alameda and Santa Clara Counties, at various sites within the organization. The CHW III acts as a liaison between primary care, specialties, and community resources, including housing, food, transportation, and financial assistance.

The CHW III will engage in outreach, care coordination, case management, and linkage to BACH resources and program services.

Qualifications

• High school diploma or equivalent.
• 2-4 years of experience in a community-based health care service capacity preferred.
• AS/AA in health or related field preferred.
• Bilingual preferred.
• CEC Certification, if applicable.
• CHW Certification - Recertification and/or retraining process, as required.

Responsibilities

• Coordinate and deliver training for CHW I through CHW II.
• Schedule staff for other external or online training.
• Prepare and deliver presentations.
• Assess patient's home living situation.
• Conduct housing and crisis assessments in various portals, as applicable.
• Conduct SDOH assessments and provide resources as appropriate.
• Conduct intake assessment of clients referred for case management.
• Monitor, document, and report staff productivity through internal and external reports.
• Assist in monitoring of patient referrals.
• Utilize PDSA model for continuous process improvement.
• Develop and nurture collaborative relationships with community partners.
• Adhere to HIPAA rules and regulations.
