Job summary

Bay Area Community Health is seeking a Community Health Worker III to support patients across Alameda and Santa Clara Counties, focusing on outreach, care coordination, and linking patients to community resources. The role requires collaboration with primary care and specialty teams to enhance access to services.

The CHW III will engage in housing, food, and transportation assistance, with travel to various sites.

Qualifications

• High school diploma or equivalent is required.
• 2-4 years of experience in a community-based health care setting preferred.
• AS/AA in health or related field preferred.
• Bilingual preferred; CHW Certification and/or recertification as required.

Responsibilities

• Coordinate and deliver training for CHW I through CHW II.
• Schedule staff for external or online training.
• Prepare and deliver presentations.
• Assess patient’s home living situation.
• Conduct housing and crisis assessments in various portals, as applicable.
• Promote HIPAA compliance and patient engagement.
