Job summary

Bay Area Community Health seeks a Community Health Worker III to serve patients across sites in Alameda and Santa Clara Counties. The CHW III coordinates care, outreaches, and links patients with housing, food, transportation and community resources.

You will support engagement, case management and resource navigation, ensure HIPAA compliance, and collaborate with primary care and specialists. Travel to various sites and some evenings/weekends are required.

Qualifications

• High school diploma or equivalent required or preferred.
• AS/AA in health or related field preferred.
• CEC Certification, if applicable.
• CHW Certification - Recertification and/or retraining as required.
• Bilingual abilities preferred.

Responsibilities

• Coordinate and deliver training for CHW I through CHW II.
• Schedule staff for external or online training.
• Prepare and deliver presentations.
• Assess patient’s home living situation.
• Conduct housing and crisis assessments as applicable.
• Perform SDOH assessments and connect to resources.
• Intake assessment for clients referred to case management.
• Monitor and report staff productivity and patient referrals.
• Develop partnerships with community organizations and ensure HIPAA compliance.
