Job summary

Travel to multiple sites and flexible hours including evenings and weekends are required.

Qualifications

• Bilingual preferred, able to engage with diverse patient populations.

Responsibilities

• Coordinate and deliver training for CHW I through CHW II.
• Schedule staff for external or online trainings.
• Assess patient home living situation and housing/crisis status.
• Conduct SDOH assessments and provide resources.
