Job summary

Bay Area Community Health is seeking a Community Health Worker III to serve patients across Alameda and Santa Clara Counties. You will act as a liaison among primary care, specialties, and community resources, performing outreach, care coordination, and case management.

The role involves housing and crisis assessments, SDOH screening, and linkage to resources at BACH and in the community, with some evenings/weekends and travel to multiple sites.

Qualifications

• High school diploma or equivalent.
• 2-4 years of experience in a community-based health care service capacity preferred.
• AS/AA in health or related field preferred.
• Bilingual preferred.
• CHW Certification - Recertification and/or retraining process, as required.

Responsibilities

• Coordinate and deliver training for CHW I through CHW II.
• Schedule staff for external or online training.
• Prepare and deliver presentations.
• Assess patient’s home living situation.
• Conduct housing and crisis assessments in applicable portals.
• Conduct SDOH assessments and provide resources.
• Assist in monitoring patient referrals and document productivity.
