Job summary

Suvida Healthcare is seeking a Guia to manage a patient panel within a multidisciplinary primary care team. You will coordinate preventive, chronic, and acute care needs, empowering patients to engage in healthier lifestyles and address social determinants of health.

Responsibilities include in-home visits, collaboration with care teams, and documenting interventions in the medical record. Bilingual English/Spanish is required to effectively serve the Hispanic senior population.

Qualifications

• 4-5 years of experience in healthcare setting or relevant experience.
• Expertise connecting patients and ensuring closed loop referral with community resources and governmental agencies.
• Experience managing the needs of Senior/Geriatric populations.
• Ability to work independently and collaboratively with physicians, families, and team members.
• Proficiency with EMRs, computers, mobile devices, and Microsoft Office Suite.

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Works with patients, family, caregivers, and providers to meet complex social needs.
• Promotes coordinated care among health care team members to ensure integrated services.
• Intervenes with patients and families regarding emotional, social, and financial consequences of illness.
• Documents all interventions in the patient medical record including in-home, telephonic, or texting interactions.
• Onboards patients to the Suvida model and their medical/social visits.
