Job summary

Signify Health is seeking a Clinician Field Mentor to lead and mentor clinicians in home visits and DPS-related workflows. You will perform In-home Evaluations, support new hires in their first 90 days, and provide ongoing guidance to active clinicians, with regional travel as needed.

The role emphasizes quality care, member satisfaction, and collaboration with Clinical Leaders and Operations Managers to resolve clinical and operational issues.

Qualifications

• Must have clinical background and state licensure.
• Willingness to travel in-region and locally as needed.
• Ability to sit, stand, bend, reach and lift up to 20 pounds.

Responsibilities

• Complete In-home Visits (IHEs, Focused Visits, Video Visits).
• Provide 1:1 mentorship to new clinicians in first 90 days.
• Offer ongoing support to active clinicians via webinars and office hours.
• Serve as first line of clinical inquiries before escalation.
