Job summary

Signify Health in the United States seeks a Clinician Player Coach to mentor clinicians, perform In Home Evaluations (IHEs) and visits, and support clinical workflow changes. You will lead by example, helping teams deliver high-quality member care and navigate new diagnostic services while maintaining KPIs.

Responsibilities include mentoring new hires, providing 1:1 coaching, and supporting active clinicians through webinars and office hours, with a focus on California and regional travel as

Qualifications

• Minimum 6 months to 1 year of experience as a high-performing Signify Health Clinician.
• Preferred multi-state licensure.
• 5-7 years of clinical experience in primary care or chronic disease management.
• Ability to travel regionally and locally as needed.
• Ability to sit, stand, bend, reach and lift up to 20 pounds.

Responsibilities

• Complete In-home Visits, IHEs, Focused Visits and Video Visits.
• Provide dedicated mentoring to new hire clinicians in their first 90 days.
• Offer group support to active clinicians on DPS updates and features.
• Serve as first line of response for clinical inquiries before escalation.
• Maintain DPS training, supplies, and adherence to workflows.
