Job summary

Signify Health, Inc. is seeking a Clinician Player Coach in California to mentor clinicians and lead in-home visits, including IHEs and video visits. You will support new hires in their first 90 days and provide ongoing guidance to active clinicians while addressing clinical inquiries and coordinating with leadership.

The role emphasizes delivering high-quality patient care, maintaining regulatory compliance, and contributing to DPS and workflow enhancements as an early adopter.

Qualifications

• Clinical background with State Licensure (e.g., MD/DO, NP/PA).
• Preferred: multi-state licensure.
• Minimum 6 months–1 year of experience as a Signify Health Clinician.
• Ability to travel regionally and locally as needed.
• Willingness to mentor and coach colleagues.

Responsibilities

• Complete In-home Visits including IHEs, Focused Visits and Video Visits.
• Mentor new hire clinicians during their first 90 days and provide 1:1 guidance.
• Provide ongoing mentorship to active clinicians and share best practices.
• Serve as first line of response for clinical inquiries and escalate when needed.
• Collaborate with Clinical Leaders and Operations Managers to improve care.
