Responsibilities

• Ensure participants gain access to social services, wellness education, arts and culture, and recreation activities.
• Create and manage a monthly calendar of activities, services, and events.
• Develop and maintain relationships with community partners and consultants.
• Develop individualized service plans and reassess regularly.
• Advocate for clients' rights, entitlements, and benefits.
• Submit weekly Dashboard and Service Log reports and maintain documentation.
