Job summary

Goddard Riverside in New York, NY is seeking a compassionate Case Manager for Phelps House to support senior residents with social services, access to benefits, housing and wellness resources. You will coordinate services, advocate for residents, and maintain accurate records in accordance with agency policies.

Qualifications

• Bachelor's degree required; preferred fields include psychology, human resources, social work, and/or sociology.
• Minimum of one year of experience in case management, social services, or working with older adults and leading groups.
• Knowledge of HRA benefits, SNAP, Medicaid, and other public assistance programs.
• Proficiency in Microsoft Office and basic technology applications.
• Ability to work independently and collaboratively within a multidisciplinary team.

Responsibilities

• Provide comprehensive case management and social service support to senior residents.
• Assist residents with navigating HRA systems and applying for public benefits (SNAP, Medicaid).
• Connect residents with community resources and outside service providers.
• Provide basic technical assistance to seniors with devices and online applications.
• Develop and facilitate educational, recreational, and wellness groups.
• Conduct assessments and maintain accurate case notes and documentation.
• Advocate on behalf of residents and coordinate with staff and partners.
• Support residents in maintaining independence and quality of life.
• Attend team meetings, trainings, and program activities.
• Maintain confidentiality and uphold ethical standards.
