Qualifikationen

• Master’s Degree in Social Work, Psychology, Counseling, or other related social service field required.
• Two years of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Strong oral and written communication skills.
• Valid driver’s license required. If driving company vehicles, must meet company insurance requirements and complete a provided driver training course.

Aufgaben

• Assess veterans referred to the program across housing, employment, education, financial, Sobriety Support/Addiction Treatment, legal, family/social, and medical/psychiatric needs.
• Coordinate care with VA personnel and community partners for eligibility documentation and health services.
• Facilitate client intake process and compile client files with complete eligibility documentation.
• Collaborate with clients to develop Individual Action Plans (IAP).
• Provide guidance to Case Manager I & II staff on client IAPs and discharge planning during meetings.
• Conduct crisis intervention and support staff with client issues.
• Maintain confidential client files in accordance with HIPAA and organizational standards.
• Document progress notes in DAP format and review updates monthly.
• Complete timely discharge summaries and conduct home visits for case management.
• Assist with program reporting, data collection, and staff training.
