Job summary

WestCare GulfCoast-Florida Inc. seeks a Lead Case Manager to coordinate advanced case management for clients with substance use and mental health needs.

You will serve as a resource for complex cases, collaborating with counselors, clinical teams, and external agencies to ensure recovery-oriented, compliant care. The role emphasizes community partnerships, individualized treatment planning, thorough documentation, and active participation in staffing discussions to strengthen service delivery

Qualifications

• Bachelor's degree in a human services or related field required.
• At least one year of professional experience providing case management.
• CAP preferred.

Responsibilities

• Manage complex cases involving co-occurring disorders and/or legal involvement.
• Coordinate with internal teams, social service providers, and external agencies.
• Advocate for client access to detox, MAT, housing, and employment resources.
• Develop individualized treatment plans based on client strengths and needs.
• Prepare accurate documentation and reports for stakeholders and funders.
• Ensure adherence to HIPAA, licensing standards, and funding regulations.
