Qualifications

• Bachelor’s degree in a human services or related field.
• At least one year of professional case management experience.
• CAP certification preferred.

Responsibilities

• Manage complex cases with co-occurring disorders and/or legal involvement.
• Coordinate services with internal teams and external agencies.
• Advocate for client access to detox, housing, and MAT.
• Develop individualized treatment plans based on client needs.
• Document accurately for internal and external stakeholders.
• Ensure HIPAA compliance and accurate EHR entries.
• Provide crisis support and community outreach.
