Job summary

Union of Pan Asian Communities (UPAC) in San Diego seeks a Lead Case Manager for the Alternatives to Incarceration program. The role conducts outreach, supervises peer support, manages cases, provides crisis intervention, and navigates systems for Re-entry Hub participants co-located at McAlister’s Recovery Bridge Center.

Ideal candidates must be registered with the Board of Behavioral Sciences, have at least two years of experience with homelessness and criminal justice-involved clients, and

Qualifications

• Must be registered with the Board of Behavioral Sciences.
• Experience with case management and homeless services preferred.
• Ability to work independently and lead a small team.

Responsibilities

• Provide comprehensive assessment of participant needs for case management.
• Create individualized treatment plans and update regularly.
• Assist clients in accessing services (housing, food, healthcare, legal aid).
• Provide short-term case management at the Re-entry Hub.
• Engage with community partners to accept referrals and link services.
• Work with an interdisciplinary team and attend meetings.
• Utilize Motivational Interviewing, Housing First, Harm Reduction and Trauma-Informed Care.
