Job summary

Amity Foundation in Los Angeles is seeking compassionate Intensive Case Managers to support individuals re-entering the community. You will assess needs, develop treatment plans, and coordinate housing, mental health, and substance abuse resources.

This role involves crisis management, advocacy for benefits, and collaboration with multiple partners to prevent reincarceration and promote housing stability. The position requires a Bachelor’s in behavioral health or a related field (or equivalent

Qualifications

• Bachelor's degree in behavioral health or social science required or equivalent with 3 years' experience in outreach, reentry services or case management.
• Strong organizational and communication skills; ability to work with diverse populations.

Responsibilities

• Interview and assess participant needs through referrals.
• Conduct wellness checks and provide crisis management.
• Develop treatment plans with participants.
• Assist with public benefits advocacy and linkage.
• Coordinate housing, mental health, substance abuse resources.
• Collaborate with housing staff, probation officers, psychiatrists and others.
• Attend inter-agency meetings to coordinate services.
• Maintain concise case records and documentation.
